Department of Transportation Exams

At Devine Family Care Practice, we ensure that interstate commercial motor vehicle drivers comply with the Federal Motor Carrier Safety Administration (FMCSA) requirements. In order to operate a vehicle, drivers must possess a valid Medical Examiner’s Certificate (MEC), Form MCSA-5876. Additionally, as part of the required physical examination, drivers must complete the medical history section of the Medical Examination Report (MER), Form MCSA-5875.

To streamline the process, please ensure that the following two forms are completed prior to your visit:

Form 1: Medical Examiner’s Certificate (MEC), Form MCSA-5876
Form 2: Medical Examination Report (MER), Form MCSA-5875
